Signs of bruxism

Why bruxism should be treated

The forces of night-time clenching are far greater than those of chewing. Over the years they wear away enamel, crack teeth, break fillings and restorations, and strain the jaw joint. Treatment does not stop the habit overnight, but it protects the teeth and muscles and eases the symptoms. The causes are often linked to stress, sleep and the bite, so the dentist may also recommend seeing another specialist – for example if sleep apnoea is suspected.

Night guard for bruxism

A night guard is a hard, clear splint made individually from an impression of your teeth. It is worn during sleep: the teeth clench on the guard instead of on each other, so the guard takes the wear. It does not treat the cause, but it is the surest protection for your teeth and for expensive restorations such as crowns and veneers.

How it is made: at the first visit the teeth, the joint and the muscles are examined and an impression is taken; at the second the guard is fitted and the bite checked. It is adjusted at check-ups if needed.

Botox for bruxism

For heavy clenching, botulinum toxin (Botox) placed in the jaw muscles temporarily weakens their contraction. This reduces the force of clenching, the tension and the muscle pain. The effect builds over days to about two weeks and is temporary – usually several months, after which the procedure can be repeated. Botox does not replace the night guard; it adds to it.

The procedure takes a few minutes and uses a very fine needle. Botox is not suitable during pregnancy and breastfeeding, with some neuromuscular conditions, or with an infection at the injection site – the dentist will ask about these beforehand.

How treatment works – step by step

1. Examination. The dentist looks for signs of wear, checks your bite, jaw joint and jaw muscles, and asks about your sleep and symptoms.
2. Plan. You discuss a night guard, Botox or both, and restoring teeth that are already worn.
3. Impression for the guard, and fitting the guard at the next visit.
4. Botox (if needed) – at the same visit or a separate one.
5. Check-up. After a few weeks you review the effect; the guard is adjusted, and Botox is repeated when it wears off.

Care and habits

• Rinse the guard in lukewarm water, clean it with a soft brush and keep it in its case.
• Wear the guard every night – it protects only while it is in your mouth.
• During the day, notice whether you clench; at rest your teeth should not touch.
• Cut down on caffeine and alcohol in the evening, and pay attention to your sleep.
• After Botox, do not massage the area or do heavy exercise on the day of the procedure.
