Aligners or braces: which should you choose?
Aligners are a good choice for mild to moderately crooked teeth when you want an almost invisible treatment and can wear them about 22 hours a day. Braces are more predictable in complex cases and do not depend on you wearing the appliance, and which one suits you is decided after an examination, X-rays and a treatment plan.

What are aligners and how do they work?
Aligners are a series of clear plastic trays made from a digital model of your teeth. Each tray is slightly different from the one before and moves the teeth by a fraction of a millimetre. The American Association of Orthodontists says the trays are worn at least 22 hours a day and each one for about a week before moving on to the next. Your dentist sets the exact schedule.
Before treatment starts, a plan is made with a simulation of how the teeth will move. With K Clear this is a 3D simulation, and with Clear X a 4D simulation with Motion Viewer. This lets you see at the consultation what the treatment can achieve and how many trays it will take.
What are braces?
Braces are small metal or ceramic brackets bonded to the teeth and connected by a wire. The orthodontist adjusts the wire at check-ups, usually every 6 to 8 weeks according to the NHS. Treatment with fixed braces often takes 1 to 2 years, and can take longer in complex cases.
The advantage of braces is that they work continuously. You cannot forget to put them in and you do not lose days of treatment. The downside is that they are visible, harder to clean and require changes to what you eat, because hard and sticky foods can knock a bracket off.

When are aligners a good choice?
Aligners give good results in cases the dentist can plan predictably. Most often these are:
- mild to moderate crowding;
- small gaps between teeth;
- relapse after earlier orthodontic treatment, when the teeth have shifted again;
- some bite problems, if the plan shows they can be corrected with trays;
- preparation before cosmetic treatment, for example before veneers or bonding.
The most important condition is discipline. If the trays spend half the day in their case, the teeth do not move as planned and the next tray does not fit. The treatment then takes longer or the trays have to be remade.
When are braces the better option?
In more complex cases braces are often the safer option. These include badly rotated teeth, a large mismatch between the upper and lower jaw, teeth that need to be pulled down or straightened over a long distance, and cases that need precise movement of the roots. Braces also suit patients who know they will not wear a removable appliance for enough hours.
This is not an absolute rule. Some of these cases are now treated with aligners too, but the plan has to show it. So do not choose the method from an advert or from what helped a friend, but from the result of your examination.
Do aligners or braces hurt more?
With both methods the teeth are moving, so it is normal to feel pressure and tenderness when chewing for the first 2–3 days after a new tray or after the braces are adjusted. With aligners this feeling is usually milder and shorter, because each tray moves the teeth only a little. With braces the metal parts sometimes rub the cheeks and lips in the first weeks. If the pain is severe or lasts more than a few days, call your dentist.
How long does treatment take?
The duration depends on the case, not only on the method. Minor corrections with aligners can take about 3 months, and more complex plans 12 months or more. What happens after treatment ends?
Once the teeth reach their final position, the bone and tissues around them need time to settle. That is why a retainer is worn after both aligners and braces. It can be removable (a clear tray worn mainly at night) or fixed (a thin wire bonded to the inside of the teeth). Without a retainer the teeth gradually move back towards where they were.
How do you choose between aligners and braces?
Before deciding, ask your dentist these questions:
- Is my case suitable for aligners and what does the simulation show?
- How many trays and how many months will it take?
- How many hours a day do I need to wear the trays and what happens if I miss some?
- What retainer will I wear afterwards and for how long?
- Are there teeth that need treatment before we start (decay, gums)?

Are aligners suitable for adults?
Yes, aligners are often used for adults because they are barely visible and come out for meals. The condition is that the teeth and gums are healthy. Before starting, the dentist checks for decay and gum problems and treats them if needed.
Can I eat with aligners in?
No. The trays come out for every meal and for any drink other than water. After eating, brush your teeth before putting the trays back, otherwise food is pressed against the teeth. That is why the time without trays should stay under about 2 hours a day.
Do I need a retainer after aligners?
Yes. After any orthodontic treatment teeth tend to move back, so a retainer is worn. It can be a removable tray worn at night or a fixed wire on the inside of the teeth. The type and how long you wear it are decided by your dentist.
How do I know if my case is suitable for aligners?
