What is a dental crown and when do you need one?
A dental crown is an artificial "cap" that covers the whole visible part of a tooth and is cemented onto it. It is made in a dental laboratory from an impression or a digital scan, and it restores the tooth's shape, size and colour while protecting it from further damage.
A crown is usually recommended when:
- the tooth is badly broken down by decay and a large filling will not withstand chewing forces;
- the tooth has had root canal treatment and is weakened, especially a back tooth;
- a fracture or crack affects a large part of the tooth;
- a bridge replacing a missing tooth needs to be anchored;
- the tooth is badly changed in shape or colour and gentler methods are not suitable.
The UK's National Health Service (NHS) describes a crown as "a type of cap that completely covers a real tooth", usually made from metal, ceramic or porcelain fused to metal (NHS – Dental treatments).
What is the difference between a zirconia and a metal-ceramic crown?
A metal-ceramic crown has a metal base (framework) onto which the dental technician fires layers of tooth-coloured ceramic. The metal gives strength and the ceramic gives the natural look. A zirconia crown is made from zirconium dioxide, a metal-free ceramic. A "full-contour" crown is made entirely of zirconia, milled from a single block.

Which type of crown is better for front teeth?
For front teeth most patients and dentists prefer metal-free materials such as zirconia. Here every difference in colour and translucency shows when you smile. With metal-ceramic, the metal base does not let light through, and over time, if the gum recedes, a dark line may appear along the edge of the crown.
That does not make metal-ceramic a "bad" choice. For some people the visible area is small, the gum is stable and the difference is barely noticeable. The dentist will judge with you how important appearance is for the particular tooth.
Which crowns are stronger for back teeth?
Back teeth take the greatest chewing load. Both materials are used successfully there. Metal-ceramic has been used for decades and its behaviour is well known. Monolithic (full-contour) zirconia is very strong and has no ceramic layer that could chip, so it is often offered to patients who clench or grind their teeth.
If you have bruxism, tell the dentist before treatment. Heavy clenching stresses any crown, and a night splint may be needed to protect both the crowns and your natural teeth.
How long do zirconia and metal-ceramic crowns last?
There is no exact lifespan that applies to everyone. How long a crown lasts depends more on the tooth underneath, your oral hygiene, your bite and regular check-ups than on the material itself. With good care many crowns last for many years, but we cannot promise a specific number of years.
The most common reasons a crown has to be replaced are decay at its margin, gum inflammation, fracture of the tooth underneath or chipped ceramic. That is why regular tartar removal and check-ups matter as much as the choice of material.
How is a crown fitted?
- Examination and plan. The dentist examines the tooth, orders an X-ray if needed and explains the options.
- Preparation. Under local anaesthetic the tooth is shaped to make room for the crown. If the tooth is badly broken down, a core is sometimes built up or a post placed first.
- Impression and temporary crown. An impression is taken, and the tooth is protected with a temporary crown until the permanent one is ready.
- Try-in and cementation. The crown is checked for colour, shape and bite, adjusted if needed and permanently cemented.
The process usually takes at least two visits, because the crown is made in a dental laboratory. The exact number of visits depends on your case.

What if a tooth is missing: crown or bridge?
A bridge is a fixed structure made of several joined crowns. It replaces a missing tooth by resting on the neighbouring teeth, which are prepared and covered with crowns. A bridge can be metal-ceramic or zirconia, and the same differences described above apply.

It is best to decide on the material after the dentist has seen the tooth, not on price alone. Sometimes the same patient has zirconia crowns at the front and metal-ceramic at the back, and that is perfectly normal.
